This third session in the “Story Series” will focus on the transition from living at home to life in medical and care facilities, as well as end-of-life care. Dr. Taku Yamada will share insights based on his on-site experience in the Netherlands.

As in previous sessions, we will explore life beyond home care and the final chapter of life through the case study of “Mrs.A.” In addition, at the beginning of the seminar, we will introduce the activities of the organizing body, Nichiran Net and Furusato Huis.

Dr. Taku Yamada

Dr. Taku Yamada holds medical licenses in both Japan and the Netherlands. Although he was raised in central Yokohama, he moved to Okinawa for his studies. After completing his clinical training, he worked for two years as the sole physician (often likened to “Dr. Koto”) on Kohama Island, a remote island in Okinawa Prefecture, serving at the island’s only clinic.

He provided comprehensive medical care to patients of all ages—from infants to the elderly—and across a wide range of fields, from internal medicine to emergency care. His experience in remote island medicine sparked a deep interest in perspectives on life, death, and health. Seeking further learning, he relocated to the Netherlands in 2019, where he studied Dutch Studies at Leiden University and later conducted research—primarily on euthanasia—at its graduate medical school.

After graduating, he worked as a medical interpreter and obtained his Dutch medical license in 2024. He currently works as a physician at an elderly care facility in The Hague, while also contributing to the Japanese community through initiatives such as the Furusato Huis Project.
